The Community Health Needs Assessment (CHNA) process, described in detail through the rest of this assessment, served to identify the priority health needs for the 75,000 individuals living with Southwestern Vermont Medical Center’s (SVMC) service area. Through five community forums, a digital survey, insight from subject matter experts, and secondary data, a variety of health needs were identified and researched. These needs were scored and prioritized using a framework that considered the scope and severity of the need, SVMC’s ability to impact the need, and the existence of evidence based interventions that can affect change to meet the need.

The Community Health Needs Assessment (CHNA) was designed to view the health conditions and status of Addison County in order to increase accountability of health care providers to their service populations, ensuring that the health care organizations’ range of services addresses local needs and promotes community health. The expectation is that health care organizations will use results from the CHNA to guide their strategic planning and resource allocation for the next 3 years.

During the period February through July 2021 an assessment of Community Health Needs in the Mt. Ascutney Hospital and Health Center service area was completed by in partnership with Dartmouth-Hitchcock, Alice Peck Day Memorial Hospital, Valley Regional Healthcare, New London Hospital, Lake Sunapee Region VNA & Hospice, Visiting Nurse and Hospice for VT and NH, and the New Hampshire Community Health Institute. The purpose of the assessment was to: Better understand the health-related issues and concerns impacting the well-being of area residents; Inform community health improvement plans, partnerships and initiatives; and Satisfy state and federal Community Health Needs Assessment requirements for Community Benefit reporting.

This report presents the findings of a comprehensive 2021 Community Health Needs Assessment (CHNA) for residents of Windham County and surrounding towns within the Brattleboro Memorial Hospital (BMH) service area. It identifies significant health needs (SHNs) in our community and establishes priorities that the BMH Senior Leadership Team has chosen based on an analysis of the findings. An Implementation Plan will be developed in the coming months to address the established priorities.

This report presents the findings of a comprehensive 2021 Community Health Needs Assessment (CHNA) for residents of Windham County and surrounding towns within the Grace Cottage Family Health & Hospital service area. It identifies significant health needs (SHNs) in our community and establishes priorities that the Grace Cottage Medical Executive Team and the Leadership Team has chosen based on an analysis of the findings. An Implementation Plan will be developed in the coming months to address the established priorities.

Rutland Regional Medical Center, as required by federal regulations, conducts a Community Health Needs Assessment every three years. The 2021 assessment is complete and the implementation strategy is in development.

Managed by the Rutland County Health Needs Assessment Steering Committee, these Needs Assessments include a broad range of data and input from within the hospital and around the community and region. Information includes secondary data from other sources as well as individual input through consumer surveys and through focus group meetings. All of the information gathered is reviewed and prioritized by a broad, community-based committee.
